Introduction

The All-India Institute of Medical Sciences (AIIMS), in partnership with IIT Delhi, has introduced AI-powered simulators designed specifically to train and assess aspiring neurosurgeons in the intricate techniques of minimally invasive microscopic and endoscopic surgeries. This innovation aims to revolutionize the evaluation of neurosurgical skills, particularly those critical to performing precise actions that significantly impact brain surgery outcomes.

Traditionally, neurosurgeons have relied on an apprenticeship model, learning through observation and assistance in operating rooms under expert guidance. However, this approach faces challenges such as limited efficiency in training and the increasing complexity of surgical procedures. Dr. Ashish Suri, principal investigator at AIIMS Delhi's neuro-engineering facility, emphasizes the need for effective alternatives to the apprenticeship model. He underscores the role of intelligent surgical simulators in providing standardized training and objective assessment of surgical skills.

"In the current apprenticeship model, neurosurgeons learn by observing and assisting an expert inside the operating room," explains Dr. Suri. "To address the limitations of this model and deliver effective training, the use of intelligent smart surgical simulators is a promising alternative."

Central to these simulators' efficacy are advancements in 3D printing and artificial intelligence (AI). 3D printing enables the creation of highly detailed, patient-specific brain models, allowing surgeons to practice procedures on anatomically realistic structures. Meanwhile, AI algorithms, leveraging deep learning and machine learning techniques, analyze a surgeon’s performance during simulations. They provide real-time feedback on precision, speed, and decision-making abilities, crucial aspects of neurosurgical proficiency.

Dr. Srinivas, the director of the institute, views this initiative as transformative for future surgeons' education, potentially altering the teaching paradigm in the field of neurosurgery. Prof. Prem Kumar Kalra, head of the computer science department at IIT Delhi and a collaborator on this project, highlights the significant strides made in automating skills evaluation using computer vision and AI/ML technologies.

"The collaboration between IIT Delhi and AIIMS through this project has yielded significant results in automating skills evaluation in microscopic and endoscopic neurosurgery training," Prof. Kalra asserts. "This advancement will further propel research into technological innovations in medical education and training."

Echoing these sentiments, Prof. Subodh Kumar from IIT Delhi underscores the potential of integrating such simulators and their objective metrics into medical curricula. He envisions a revolutionary impact on medical training, with standardized evaluations enhancing the proficiency and readiness of future neurosurgeons.
